These are hospitals where a physician has privileges (where they have the hospital's permission to utilize the facility to care for their patients). This information is updated by the health plan at the time of the credentialing/re-credentialing process at least every three years, and when the physician notifies the health plan of any changes.

A physician’s affiliation with a hospital does not guarantee the hospital is a participating provider in your plan’s provider network. Please confirm your desired hospital is in-network.

Accepting New Patients indicates if a physician is currently accepting new patients into their practice.

Physicians self-report if they are accepting new patients or not at the time of initial credentialing and through annual updates thereafter. Limitations to this information may occur if the physician does not inform PacificSource. Requests to update and verify this information are sent to physicians annually.

Providers self-report their gender:
Man, Woman, or X (a gender-neutral marker).

Some plans or networks have more than one level of participating providers. You’ll receive the most value by seeing a Tier 1 provider in your plan’s network.
